First Responder and Medical Directive Envelope

Medvelope Alert Systems LLC is proud to offer the Medvelope™, a clear plastic packet that is affixed to a patient's door, containing pertinent medical information for first responders, emergency medical technicians, and other medical personnel. The Medvelope™ packet includes a Velcro™ strip to fasten the Medvelope™ package to the back of the residence door; two zip ties so the patient can attach the packet to another location, such as the doorknob, the stairs by the front door, or other highly visible location. The Medvelope™ also comes with two window stickers to be placed next to the front door or back door of the house, or directly on the front and/or back door. The sticker alerts EMTs and first responders that a Medical Directive for Health Care is located in the residence. This is very important so that first responders will know to look for the Medvelope™, especially when no one is available to direct them to the document.

Please note - Medvelope™ is not a Medical Directive; it is a critical safe place to store documentation that is filled out by the patient and/or a healthcare provider and attorney1.

Medvelope™ was designed and developed to solve the problem of emergency first responders having accurate medical information about their newest patient immediately upon arrival in critical emergency situations. Medvelope Alert Systems LLC has worked very closely with numerous health professionals, including doctors and other emergency personnel, to develop this sample EMT medical directive packet. The ample storage room of the Medvelope™ allows it to accommodate physician or health provider-supplied information for a first responder medical directive, as well as important information, including:

  • Rescue Medical Directive
  • Power of Attorney; Durable Power of Attorney for Health Care Directive
  • Emergency Medical Directive (DNR – Do Not Resuscitate; or Authorization to provide medical assistance, including CPR (cardiopulmonary resuscitation), medications, medical procedures, etc.)
  • Emergency Contact Information (Physician/Medical Specialist, Family, Friends)
  • Medications List the patient is currently using including an Allergies List
  • Life Flight Medical Directive
  • Health Insurance Information
  • Living Will Medical DirectiveFirst Responder and Medical Directive Door Hanger
  • In Case of Emergency contact information
  • Door Hanger, This directs the EMT’S to the location of your medical information if you chose not to hang the MEDVELOPE on your front door.

Documents that are placed in the Medvelope™ need to be reviewed and approved by a physician, healthcare provider, and/or attorney. In some states, emergency medical directive documents must be notarized. Consult with your attorney and physician for further advice and information.
